Social Prescriber

Social Prescriber Roisin Reilly, can help explore with patients those local activities and services that could improve their wellbeing; ‘what matters to me’.

Community services include healthy lifestyle and activities, benefit and financial support and advice, employment and volunteering, education and learning, befriending, getting involved in local groups and activities plus accessing specialist services. There is always a follow up contact with a patient to check that agreed goals have been initiated.

Any staff member of the surgery can make a referral to Roisin.
